In-Office Capabilities
When clinically appropriate, we can perform common tests and minor procedures in the office—helping you get direction and treatment efficiently.
Included Membership Benefit
When a clinical question would benefit from another level of expertise, your provider can request focused guidance from an appropriate specialist—often within 24–48 hours—and bring that wisdom back into the context of your complete health story.
The real advantage is not simply access to a specialist. It is receiving specialist insight through the provider who already knows your history, medications, goals and circumstances.
How Specialist Insight Becomes Your Care Plan
We listen, evaluate and identify the clinical question that needs deeper expertise.
Your provider sends the relevant history, records and a clear question to the appropriate specialist.
The specialist provides focused recommendations based on the information provided.
Your provider explains what the guidance means and applies it to your overall care.

Focused recommendations often arrive within 24–48 hours instead of waiting weeks or months for initial guidance.
Specialist input may prevent an unnecessary appointment, trip or added expense—or confirm when a referral is truly needed.
Your primary care provider remains involved, interpreting the recommendation alongside the rest of your health story.
Access to over 100 specialties: Our e-consult network connects your provider with expertise across more than 100 medical specialties, depending on the clinical question and appropriate specialty match. If an in-person specialist visit is still needed, we will explain why and help guide the next step.
E-consults are included for members when initiated by a Lafayette Family Wellness provider. Patients do not meet directly with the consulting specialist through this service.
Additional Costs
We recommend maintaining health insurance for major, unexpected or hospital-based care. DPC membership is not insurance.
What Happens Next
Here is what to expect from application through your first appointment.
Tell us about your healthcare needs and what you are looking for in a primary care practice.
Applying does not establish membership, and no payment is processed at this stage.
A staff member will contact you within 3 business days to schedule a discovery call to discuss expectations and answer all your questions. We do this to ensure your needs are taken care of.
If our services are a good fit for your goals, we will process the registration fee and first membership payment. We will request health records and set up your communication app.
Once enrollment is complete, we will schedule your first provider visit. New-patient visits are typically available within 2–3 weeks and are generally scheduled for one hour.

Your First Appointment
Your first provider visit is typically one hour. The goal is not to rush through a checklist, but to understand you, your health history and the concerns that matter most.
A True Care Partnership
Strong primary care works best when patients and the care team communicate openly and participate together.
